Hi, I’m Hannah Kernen— a Certified Dementia Practitioner and founder of Dementia Dimensions. I understand that caring for your loved one with dementia can feel overwhelming and isolating, however I have the expert guidance, tools and resources to help you.

My services are focused on improving your whole family’s quality of life. Not only do I offer strategies to support those with cognitive challenges, but I also give you the tools you need to prioritize your own wellness as a caregiver.

You won’t find any “one-size-fits-all” approaches to dementia care here. I will get to know your family and provide tailored support to help you navigate your unique challenges. Hi, I'm Hannah! I’m a Certified Dementia Practitioner and founder of Dementia Dimensions.

Dementia help may seem difficult to find and access for you and your loved one. Many families are unable to find answers and solutions to the challenges dementia has brought into their lives, and doctor appointments often feel short, allowing limited discussions regarding dementia support. I founded Dementia Dimensions to provide much needed support to families, caregivers, and individuals with dementia where everyone feels heard and supported.

My approach is to listen first, provide resources and tools for now and for the future, and educate care partners about the course of dementia— including physical, emotional, and cognitive challenges that may impact the whole family. I am passionate about making you feel at ease and confident in your decisions as a caregiver.

I use my five plus years of experience from working in skilled nursing facilities, memory care facilities, assisted living facilities, long-term acute care hospitals, and private homes to help my clients navigate dementia and the healthcare system. No matter where you are in the journey, I promise to provide compassionate care that puts your quality of life above all else.
